Edelweiss Home will give care to residents who are:
n ambulatory or non-ambulatory (including high fall risk).
n transfer dependent (including bedridden residents requiring total transfer assistance).
n terminally ill residents (including end of life care).
n with cognitive impairment.
n with hearing or visual impairment.
n with dietary limitations or restrictions (diabetes, pureed diet, thickened fluids, etc.).
n requiring Incontinence Management.
n requiring Oxygen and/or Catheter.
The following basic services are available to all residents of Edelweiss Home:
n Three nutritious meals and between meals nourishment or snacks.
n Special diets if prescribed by a physician.
Help with all activities of daily living such as:
n Assistance with bathing.
n Assistance with toileting.
n Assistance with dressing.
n Assistance with grooming.
n Assistance with eating.
n Assistance in meeting necessary medical and dental needs.
n Assistance with taking prescribed and over-the-counter medications in accordance with physician’s instructions unless prohibited by law or regulation.
n Bedside assistance and tray service for temporary illness or recovery from surgery.
Edelweiss Home will also provide:
n Comfortable and suitable bed and bedroom furniture.
n Fresh bed and bath linens weekly or more often if necessary.
n Hygiene items of general use such as soap and toilet tissues.
n Laundry Service.
n Cleaning of resident’s room.
n A planned activity program including arrangement for utilization of available community resources.
n On-going care and supervision, and observation for changes inphysical, mental, emotional and social functioning.
n Notification to resident’s family, physician and other appropriate person/agency of resident’s needs if there is a significant change in condition.
Additional Services (like Transportation to doctor’s appointments or Third Party Services) are available on request.
